Office Policy & Consent

• I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, and improvement of circulation and energy flow.

• I understand that the services offered today are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.

• This time has been reserved for you. If you are unable to keep your appointment kindly give 24-hour notice. Should you not show up or call to cancel, you'll be charged the full amount of your scheduled session.

• Clients are asked to provide a complete and accurate health history, and notice of health changes at successive appointments.

• I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so. I affirm that I have notified my therapist of all known medical conditions and injuries.

• If the client is late, the session ends at the time originally scheduled, and the full payment is to be paid in full.

• If the massage therapist is late, the session is to last the full amount of time originally scheduled.

• Feedback is encouraged. Please inform the therapist of any unusual sensation or discomfort, so that the application of pressure, stroke, and hot or cold therapies may be adjusted to your level of comfort.

• The unclothed body is properly draped at all times for your comfort, and sense of security and as a mark of massage professionalism.

• written referrals and/or approval for massage therapy may be requested from your primary caregiver if you are currently under a doctor's care, or have a specific medical condition, and/or symptoms for which you take medication, and or receive periodic medical treatment.

• Full payment is due when services are rendered.

• Clients with a High 'NO SHOW' history are to prepay for their appointments by cash or credit card only.

• Gift certificates are transferrable but non-refundable, and expire one year from the date of purchase.

• Discount cards cannot be used toward the purchase of gift certificates or any other promotions.

• No Double discounts regarding occasional coupons or specials.

• No refunds are given for services rendered.

• Prices are subject to change without notice.

• Cash, Visa, Mastercard, American Express, and Discover are accepted.

• Checks are NOT accepted. Sorry for the inconvenience.

• During the initial consultation, focused attention and massage therapy are given as agreed upon by the therapist, and the client for the predetermined goals of stress reduction, relief of muscular discomfort, and/or health promotion. You have the opportunity to ask questions.

• THERE IS NO SEXUAL ASPECT TO MASSAGE THERAPY: If the therapist deems your behavior inappropriate, the session is to end immediately, and payment in full is to be made for the scheduled session.

• APPROPRIATE PROFESSIONAL BEHAVIOR: Our profession ascribes to a code of ethical behavior. I agree with all the statements in this ethical code and have strong beliefs that the therapist, and their clients should NOT engage in intimate relationships.

• By signing this release, I hereby waive and release my therapist from any and all liability, past, present, and future relating to massage therapy and bodywork.
